A Minor Works certificate should tell the client what was done, where it was done, and what changed. The description should be specific enough that someone can understand the scope later without guessing.
That means clear client details, a sensible description of the work, the circuit or accessory affected, and any readings or notes that support the work carried out.

The big question is whether the work stayed as minor work on an existing circuit or became new installation work. If the job introduces a new circuit, you need to look at when an EIC is required rather than trying to force the job into a Minor Works form.
If the job includes replacing a consumer unit or distribution board, a full Electrical Installation Certificate is required regardless of whether new circuits were added (Reg 644.4.201). A Minor Works Certificate is not permitted for this work — GN3 Reg 2.5 explicitly excludes it.
The certificate should reflect the real scope of the job. If the work went beyond minor alterations on an existing circuit, the paperwork should move with it instead of staying on the smaller form.

Most Minor Works Certificate mistakes come from rushing — the certificate is short, so people treat it as a formality and finish it without checking. The result is missing supply details, incorrect circuit identification, or a declaration ticked before the test results are even in the boxes. None of those are technically illegal, but all of them weaken the legal value of the document.
This is the most commonly failed compliance point. If you altered three circuits, you must issue three separate Minor Works Certificates — one for each circuit modified (GN3 Reg 2.5). A single certificate covering multiple circuits is non-compliant.
The fix is a short pause before sign-off. After every test is in, before any signature, read the certificate end to end as if you were the client or an inspector. Does every field describe the work you actually did? Are the readings consistent with the circuit? Is the declaration honest? If any answer is "not quite", the certificate needs another two minutes before you hand it over.
