- AThe site manager, recorded in the accident/incident book and used to update the RA
- BThe person who nearly fell, who decides for themselves whether to mention it
- COnly the HSE, because near-misses are reportable under RIDDOR like injuries
- DNobody — no one was actually hurt, so there is nothing to formally report
Why: Near-misses are gold-dust learning events. Report to site management, log it, update the risk assessment so the next person is protected.
